Given that you're pretty new to this, I would suggest you proceed in stages.
1. Get data in DICOM format at scanner at end of scan. Transfer data to AFNI.
2. Get data in DICOM format at scanner at each TR. Transfer data to AFNI.
3. Analyze on remote or same computer and send something back to scanner or stimulus presentation computer - atlas-based ROIs, locator task ROIs, SVM, motion parameter based results, ...
There can be different challenges with different scanners, network configurations, information to be processed and stimulus presentation. See the class material, the rtfeedme script example, the Dimon help, the to3d help also, and some previous messageboard postings too. There is a readme about configuring realtime display in AFNI. There was also an old realtime_howto document, but I'm not sure how up to date this is.
