It turned out that the problem images weren't DICOM at all - they were the earlier SIemens-specific mosaic format. Confusingly, both types of images get the ".ima" suffix. Anyway, the existing to3d software can un-interleave such images, when the environment variable AFNI_SIEMENS_INTERLEAVE is set to YES.
