What happens if you just try to read the data WITHOUT the 3D:0:0:256:256: prefix on the filenames? AFNI should properly figure out the correct data offsets from the DICOM headers.

3D:0 means that the 256x256 data is read out from the start of the file, which is why things are wrong (that's where the DICOM header is). 3D:-1 means to read from the end of the file, which is why it is correct (since DICOM always puts the image data after all the header info).
