Reviewing your 3dVol2Surf command, it looks
like your -grid_parent dataset should not be
AM-04VSUMA_SurfVol+orig, it should be the
lh_insula_roi+orig dataset that you created
with 3dSurf2Vol. That is where the data comes
from for the output surface.
It is not clear what your -sv dataset should be,
but I'll guess that you just have the two (-sv and
-grid_parent) datasets reversed.
